I had just one night in Okinoshima, but the receptionist was incredibly kind and even drove me to a grocery store so I could pick up food. I went for a run along the bay and enjoyed the beautiful waterside scenery. The bathhouse was spotless and the water really warm. My simple Japanese tatami-style room looked picturesque with the rest of the island behind it. Such great service, and a beautiful place to stay.

The service is really good, and the price is reasonable, around $75 including dinner and breakfast. The rooms and other facilities are a bit dated, though, and the location and noise may be an issue for some. It's a 14-minute walk from Saigo Port. You can hear corridor noise easily, and kids running or talking loudly can be bothersome. It seems to host groups more often than individuals, so people chatting in the hall and going between rooms can make it noisier. There are some good extras: a clean, cozy public bath, though there is no shower or bath in the room; a free airport shuttle if booked in advance; and car rental at ¥1,000 an hour, capped at ¥5,000 a day. The staff are very kind and helpful, and there's a mountain and ocean view.
You can use the public bath and enjoy a nice view of the bay. The staff were hospitable, driving me to the city centre and putting together a map to the nearest convenience store. It's a Japanese-style hotel that works for families with children, though solo travelers can stay too.
It's an older hotel, but clean, and it has an onsen.
There's a pellet stove inside :)
